Key Elements of an Audit Report

Audit Information

The report should begin with basic audit information, including:
  • Audit Reference: A unique identifier for the audit
  • Date: When the audit was conducted
  • Audit Criteria: The standards or regulations against which the audit was performed
  • Audit Scope: The extent and boundaries of the audit

Findings Categories

To effectively categorize audit findings, the report should include the following classifications:
  • Major Nonconformity (Major NCR): Overall breakdown in management system requirements
  • Minor Nonconformity (NCR): Isolated lapse in requirements
  • Observation (O): Minute deviation from requirement but could lead to nonconformity if left unattended
  • Conformance (C): Demonstrates conformance to a requirement
  • Note (N): For the purposes of information or making a record

Findings Table

The core of the audit report is the findings table, which should include:
  • Number: A sequential identifier for each finding
  • Description: Detailed explanation of the finding
  • Criteria Reference: The specific standard or regulation related to the finding
  • Finding: The category of the finding (e.g., Major NCR, Minor NCR, Observation, etc.)
The findings table should have sufficient rows to accommodate all observations made during the audit.
